at minimum lead time from Booking to Start to account for Credentialing/ComplianceSSN DOB RTO must be included at time of the submission Can only submit to 1 location within the region (PAMC, Seward, Valdez, Saint Elias Specialty Hospital, and Kodiak) Valid State License (Allowed to submit with a pending license, takes 10-12 weeks) Mile Radius Rule Applies ( must be 50 miles from facility) References Required No Locals Accepted Candidates will float within the hospital or to affiliate hospitals near by Copy of Certifications Specific Charting Exp(Epic) Return Staff Policy (6 months to return as traveler) Facility only accepts Hep B vaccines and will not take titers or declination ALL travelers are expected to float between all facilities within a given statetravelers' refusal to float may result in termination of assignment and/or forfeiture of guaranteed hours.
If required for the assignment, Client agrees to supply Providers with communication devices (i.
ecell phone, pager, etc.
