Summary:
The Care Team Case Manager is a key member of the University of Alaska Anchorage (UAA) Dean of Students Office.
This position is a member of the UAA Care Teams and closely collaborates with the Student Health & Counseling Center, Disability Support Services, and Residence Life.
This position provides:
1) case management services, 2) updates Care Team records, 3) consults with students, faculty and staff, 4) provides support to the Dean of Students Office, and 5) provides outreach and training.
The Care Team Case Manager is a 40-hour per week, 12-month, term-funded position.
This position is for the 2023-2024 academic year.
In spring 2024, the Dean of Students Office will determine if there is funding available to renew the position.
Duties:
15 Dean of Students Office (DOSO) Support:
Provide support to the daily operations of the Dean of Students Office, to include helping with phone and walk-in traffic.
Work on DOSO projects as assigned by the Dean of Students or Assistant Dean of Students and Director of Student Conduct & Ethical Development.
Essential 5 Consultant:
Serve as a consultant to students, staff, faculty, families, and members of the UAA community regarding students of concern and their behavior to determine the most appropriate response that meets the constituents and Universitys needs.
Provide appropriate strategies and referrals to address student behavior.
Essential 5 Outreach and Training:
Help foster a culture of reporting on campus by advertising the services provided by the Care Team to the UAA Community.
Provide training to the UAA community regarding how to recognize at-risk students and how to make referrals.
Help design, plan, implement, and evaluate publicity, marketing, workshops, and trainings to promote the Care Team and its services.
Present workshops and trainings as needed.
Essential 75 Case Management:
Provide case management services for students by arranging, coordinating, monitoring, evaluating, and advocating for students.
Follow the UAA Care Team Manual.
Intake reports of students of concern.
Collect, organize, and evaluate facts associated with each case.
Communicate with student(s) and referral source(s).
Collaborate and consult with key stakeholders.
Communicate with parents, families, or designated emergency contacts as appropriate regarding significant mental health emergencies including hospitalizations, suicide attempts, etc.
Meet with students, family members, University and community members, to address needs and facilitate access to services and follow-up care.
Utilize a trauma-informed care approach.
Create action plans for each case and help students set goals to fulfill those plans.
Refer students to University and/or community services and assist them to follow-through on the referrals provided.
Help students navigate University processes and procedures.
Submit Care reports about students of concern.
Create cases, enter case management notes, enter Care Team meeting summaries, and upload records in the online database.
Participate as a member of the UAA Care Team and Residence Life Care Team, as appropriate.
Essential Knowledge, Skills, Abilities:

Possess demonstrated effective skills in organization, planning, decision-making, problem-solving, strategic thinking and the effective use of resources.

Must also have effective interpersonal, oral and written communication skills.

Ability to handle confidential information in a sensitive manner and use sound judgment in responding to crisis situations.

Knowledge of trauma and implications for advocacy.

Experience providing culturally relevant training to diverse audiences, working with diverse peoples, and creating inclusive and welcoming environments.

Ability to work with students, staff, faculty, upper level administration, parents, and other stakeholders.

Knowledge of and experience in working with adolescents and young adults.
Computer skills including presentation software, email, Microsoft Office suite, and databases.

Must be able to work flexible evening and weekend schedule, as necessary.
Preferred Experience & Education:
Three years of professional experience in case management services or related field such as student affairs or counseling.
Graduate school experience may count toward this requirement.
Required Experience & Education:
A bachelor's degree from an accredited institution in a relevant discipline such as human services, psychology, sociology, education, or social work is required.
Or an equivalent combination of training and experience.
A master's degree from an accredited institution in college student personnel, higher education administration, counseling, public health, psychology, social work or other related field is strongly preferred.
